you have to contact the local health centre you are registered with and fill in a special form indicating your preference

What happens next?
the doctor is notified and, if s/he agrees to take you on, your details are registered by the health centre; if the doctor refuses (for reasons such as work overload or patient living outside the catchment area), you will be offered another doctor
How can you register on a temporary basis?
you can register in your area of actual residence; you need to make an extra application to the head doctor of the health centre; only persons staying in the area no more than 90 days may register in this way
Under what conditions can a doctor refuse to take on a patient?
if the patient does not conform to the regulations, as long as this does not endanger the patient’s life and health; must inform the health centre and the patient
And what if the patient wishes to change doctors?
s/he has the right to do so, but must inform the centre in writing; can then register with a different doctor
